Filling Thailand Visa on Arrival Form
When applying for Visa on Arrival Thailand, you need to submit main information about your visit and personal details. The form should be filled up in English.
On the top right side of Visa on Arrival form you will need to attach your photo taken within the last 6 months.
Then you need to choose your status (Miss, Mr or Mrs) and underline the correct one. When it comes to your name, you need to write it in capital letters and Latin characters only. Pay attention – family name comes first on this form!
For the nationality, write the name of the country which passport you are using. Passport No. should be written clear and exactly the same as on your travel document. Double check it!

Flight No. must be filled with the details of your arrival flight and should be the same as on Thailand Arrival Card which also needs to be presented. Flight No. can be easily found on your flight ticket or boarding pass.
Your occupation can be described in short with the name of your present position. The name of employer isn’t required.
Permanent address is your address in your home country.
When providing address in Thailand, be sure to give not only the name of your hotel but its address as well. Fill it with details from your accommodation booking confirmation as lately they thoroughly check if details of your booking comply with details provided on Visa on Arrival form. If you haven’t booked your accommodation yet, just book any hotel on Booking.com or Agoda and fill its details on the form.
Name and address of person of reference in Thailand normally can be left blank unless you will be stating that there will be a person who will provide accommodation for you.
If you are traveling with minor children who are traveling on the same passport, you need to provide their names, birthdays and places of birth in the designated area.
Don’t forget to write the date of your arrival and to sign the Visa on Arrival form. You would be surprised how many people miss it!
The last part of Thailand Visa on Arrival form should be left for official use only and shouldn’t be filled with any information.
